Der Test kann auch im Rahmen eines Hypophysen-Stimulationstests durchgeführt werden. Insgesamt ist der TRH-Test selten indiziert.

- Frühform des M. Basedow (TRAK zu 75% positiv, hTPO zu 80% positiv, TAK zu 25 % positiv)
- Levothyroxintherapie
- HVL-Insuffizienz
Delta-TSH > 20 mU/ml, (f)T3 normal, (fT4 vermindert)
- Hinweis auf latente Hyopthyroese
- Frühstadium einer chron. Thryreoiditis
Delta-TSH > 25 mU/ml, fT4 erniedrigt, (f)T3 vermindert
- manifeste Hypothyreose
Indikation
V.a. latente Dysthyreose, Hypophysenvorderlappeninsuffizienz
Durchführung
1) erste Blutentnahme nüchtern (TSH basal),
2) 200 µg TRH langsam in ca. 2 min i.v., Bei Kinder 7 µg / kg KG
3) zweite Blutentnahme nach 30 min (TSH stimuliert),
4) Variante: statt i.v.-Applikation TRH-nasal, in diesem Fall zweite Blutentnahme nach 2 Stunden
